Palliative Care at Home, for Patients in the DFW

Palliative care is a specialty in medical care that focuses on relieving the symptoms and stress of chronic disease.  Aspen Healthcare delivers this medical care in the comfort of your home. We send nurse practitioners to the homes of our patients. Their work may also be supported by visits from a licensed social worker, MD, or DO. Please contact us now to discuss your situation.

Who Should Receive Palliative Care?

Palliative care is for patients with two or more medical conditions, a medical condition in an advanced state, and a high symptom burden. Medicare and most health insurance cover palliative care under these conditions. Unlike hospice benefits, palliative care has no prognosis requirement. Patients may receive palliative care at any stage of a disease. Patients may receive palliative care concurrently with home health services as well as while receiving medical treatment from other providers for the same diagnosis.

How Does Palliative Care Help?

At Aspen Healthcare, our palliative care team works together with your doctor to provide the level of care that you require to live your life to the fullest. According to your individualized care plan, our specialist providers spend considerable time with you - usually multiple visits per week, with each visit lasting up to an hour. With specialist knowledge and a singular focus on symptom relief and quality of life, our clinicians prescribe medications, order other services, teach palliative care strategies, provide counseling, coordinate care, evaluate progress, and continuously manage the palliative care plan until optimal quality of life is achieved. We work on your medical needs such as:

Understanding Palliative Care and Hospice

Palliative care is a category of medical specialties. However, it's also a term often used to categorize any treatment that is intended to alleviate symptoms rather than cure a disease. This can be confusing, because, as a category, all hospice care is palliative care, but not all palliative care is hospice care. Both palliative care and hospice care prove very valuable for the right patients. In short, hospice is best for patients in an advanced stage of a terminal disease, when reasonable curative treatment options have been exhausted or are no longer desired, and when a patient's goals have shifted more toward quality of life. Home visits through Aspen's palliative care program are for patients in earlier stages of chronic disease, when symptom management could benefit from more attention than is normal for other care settings. Let's breakdown the differences.
